Annual Checklist

The Office of Student Accounting offers an annual checklist designed to help answer common questions.

New students: You will need to activate your CWID before you can access the LEARN forms below.

To access the following forms, visit LEARN, “Financial Aid & Billing,” “New Students Onboarding Forms” and “General Forms.”

Electronic Attestation: Confirm required electronic acknowledgments to receive official institutional communications via your Weill Cornell issued email address.

Financial Responsibility Agreement: Review and accept the Financial Responsibility Agreement.

U.S. Social Security Number or Tax ID Update: Verify or update your SSN or Tax Identification Number for financial aid and tax reporting purposes.

Personal Information Update (PIU): Ensure your personal and contact information is current.

Direct Deposit: Set up direct deposit to receive refunds quickly and securely.

1098-T Electronic Delivery Consent: Opt-in to receive your 1098-T tuition tax statement electronically.

1095-B Electronic Delivery Consent: Opt-in to receive your 1095-B health coverage tax form electronically.

Health Insurance Waiver: Submit a waiver during the open enrollment period (link to 2.16.1.2. Waiving & Cancelling) in Gallagher Student Health if you have qualifying external health insurance coverage.

Third-Party Sponsor Application: Complete if your tuition or fees are paid by an employer, government agency, or other sponsor.
