Onboarding Forms

To complete your onboarding forms, login to LEARN, select “Financial Aid and Billing” and select “New Student Onboarding Forms.”  

Electronic Communication Agreement

All students are required to submit the Electronic Communication Agreement, which provides consent to receive official institutional communications via their Weill Cornell issued email address.

Financial Responsibility Agreement

All students are required to submit the Financial Responsibility Agreement confirming their understanding and acceptance of Weill Cornell Medical College’s financial and payment policies.

The form must be submitted during the onboarding process. Failure to submit the form will result in a hold that will prevent academic activities such as registration, transcript requests and viewing grades. 

Financial Responsibility Agreement Terms:

  • Students accept full responsibility to pay all tuition, fees and other associated costs assessed as a result of registration and/or services as they appear on the student account.
  • Failure to pay the bill or any monies due by the scheduled due date and/or failure to make acceptable payment arrangements to bring the account current, may result in a finance charge at a rate of 1.25%/month (15% Annual Percentage Rate).
  • Weill Cornell Medicine reserves the right to defer any delinquent account to a collection agency if it remains unpaid. Furthermore, a debtor is responsible for paying the collection agency fee which may be based on a maximum of 33.3% of the outstanding account balance, in addition to the amount owed.
  • A delinquent account may be reported to one or more of the national credit bureaus. 

U.S. Social Security Number or Tax ID Number Information

Submit your U.S. Social Security Number or Tax ID Number Information to verify your information so our offices can process your financial aid and issue your 1098-T tax form.

Other Forms 

1098-T Electronic Delivery Consent

Students may receive their 1098-T form electronically. By submitting this form, you consent to Weill Cornell Medicine delivering your 1098-T electronically instead of by mail to your address on file.

EMBA/MS Textbook Fee Waiver

Weill Cornell Medicine offers you the option to opt out of receiving textbooks, course packets, and other publicly available course materials. Materials that are not publicly available will continue to be provided by the program, and their costs remain included in the all-inclusive tuition.

Health Service Annual Fee Waiver

To request a waiver of the annual Health Services Fee, please email your request to the Office of Student Accounting. Complete the provided form and include all required documentation.

Eligibility for a Health Services Fee waiver is subject to specific conditions, including:

  • You are a current employee at Weill Cornell Medicine or NYP/Weill Cornell or a current employee at an affiliate (MSKCC, HSS, Lincoln, other NY campuses) with employer sponsored health insurance and anticipate remaining an employee during your enrollment as a student.
  • You are employed full-time (outside WCM/NYP or affiliates) and are unable to utilize Student Health Services during normal hours of operation (Monday - Friday 8 a.m. - 4 p.m.).
