1095-B Health Coverage Form

In compliance with the Affordable Care Act Individual Shared Responsibility provision, all individuals covered under the Cornell Student Health Plan (SHP) are legally required to provide a taxpayer identification number—either a Social Security Number (SSN) or an Individual Tax ID Number (ITIN)—to Cornell.

This number allows Cornell to submit 1095-B forms to the IRS on behalf of students enrolled in the SHP during the year. The 1095-B verifies that plan members had minimum essential coverage, as required by the ACA.

Receive Your 1095-B Form

To be eligible to receive your 1095-B form, submit in LEARN (under “Financial Aid and Billing” and “General Forms”), you and your dependent(s)’ taxpayer identification number and the 1095-B Electronic Delivery Consent Form.

Students will receive a copy of their 1095-B forms at the beginning of the spring semester. 

If you would like to receive a copy – or a corrected copy – of a previous year’s 1095-B form, please send your request by email to shp-finance@cornell.edu (and include "WCM 1095-B request" in the subject line).

  • Your request must include your full name, your student ID, an email or phone number where you can be reached, and the address where you would like the 1095-B sent. Please note the address provided for your 1095-B mailing is for the Student Health Plan only and will not change your address on file with Weill Cornell Medicine.
  • If you would like the form emailed, please provided an email address and indicate you prefer to receive your 1095-B via email.

Requests will be processed by the Cornell Student Health Plan within 30 days of receipt of the request.
